Abstract

The shone’s complex, defined by four cardiovascular defect such as supravalvular mitral membrane, valvular mitral stenosis by a parachute mitral valve, subaortic stenosis and aortic coarctation. Some define the anomaly as consisting of a congenitally abnormal MV along with 2 other obstructive lesion in the left heart or systemic circulation, is a rare entity, which occurs most frequently in its incomplete form.1 We report a case of a 5-year-old female patient presented at the emergency departement for progressively worsening dyspnea, fever, and productive cough, due to bronchopneumonia, cyanosis. Echocardiography revealed the co-existence of left ventricle and aortic hipoplastic, severe mitral stenosis due to parachute mitral valve, VSD and a large PDA . Antibiotics wide spectrums was administered since admission in emergency departement, , after nine day hospitalization pneumonia showed progression but the family refused any further investigation, treatment,  and was discharged againts medical advice.
